This helpful document provides guidance on what to expect and the services available at key stages of dementia, in a way that’s easy to understand.

It outlines:

  • The various stages along the dementia pathway from pre-diagnosis, through independent living at home to significant need for care and support and finally, if relevant, the stage following the death of a person with dementia.
  • At each stage services offered are outlined for example, GP, Memory Assessment Service, social work team, occupational therapist, clinical psychologist and Adult/Older Community Mental Health team to name a few.
  • Also aligned with each stage is the support Dementia Jersey provides which evolves as families progress on the dementia journey.
  • The document also includes useful links and a definition of dementia and common symptoms.
