Dementia Jersey Holistic Needs Assessment

Dementia Jersey is working in partnership with the Memory Assessment Service to offer free support for people who have recently been diagnosed with dementia, as well as for their main family carer.

We provide what we call a Holistic Needs Assessment. This is a relaxed and supportive meeting that gives you the chance to talk about anything that may be affecting your life and wellbeing.

One of our Dementia Advisors will meet with you to understand what matters most to you and help you find support that suits your individual needs.

We look at all aspects of your wellbeing, including your emotional, social, financial and spiritual needs — not just your physical health.

For people living with dementia:

Our focus is on helping you live well — supporting you to continue doing the things you enjoy and help you access the support that’s right for you.

For carers:

This is time for you. It’s a chance to talk about your own needs, feelings and experiences as you support someone else

  • You’re welcome to bring a relative or friend for support if it helps, choose someone you know well and feel comfortable with.
  • For carers, we recommend coming on your own so you can speak openly
